The trouble with WoW is that it is an old game now. I did enjoy it back in the day, until I realised it was essentially a second job, but I would like there to be a new high-end MMO, preferably not a high fantasy one, that I could get into from the beginning. Particularly if I could play with my 19yo son who just went off to Uni. But although there have been plenty of challengers, nothing has really stuck the landing.

Well it is an old game insofar as the oldest content in the game is now very old, but you have to hand it to Blizzard they do an amazing job of producing new content which is both in keeping with the older parts of the game, and incredibly pretty. It's an absolute credit to the original art design team. I also dip into the WoW Classic servers and that's when you really see how much it's come along in terms of visuals. But I'm a filthy casual and despite now being back again at the level cap with my main I'm still nowhere near getting into endgame content - having huge fun though.

WoW stole all my Unreal Tournament and Counter-Strike buddies Is Guild Wars 2 still going strong? Only MMO I ever played and quite enjoyed it.

GW2 still going strong, not WoW numbers, but I don't think it ever was. They had an expansion out a few weeks ago as well.

How much of the game can you access without paying? I reckon I bought every expansion up to Omens of War, but I expect my account got deleted given that I haven’t played in nearly 20 years?

There are a few options.
You can download the official launcher, which has some free to play (with restrictions) servers and subscription servers. Additionally, it can also give you access to the test server, which is free and gives you more access. Those servers have the most up to date versions of the game, with most expansions, and the premium ones also offer TLP (Time locked progression) servers which aim to replicate the original game and add each expansion in every three months or so.
If you're looking for a more original experience, there is Project 1999, which is a version of the game which I think contains the original EQ and the first two expansion packs (Kunark and Luclin?). This is free, but requires a specific version of EQ to install from (there are links to this on the internet) and to follow a set of instructions for patching and setting up a p1999 account. Its not too complicated but does take a little bit of time to do (Quadfather provided some links to the instructions earlier in this topic).

I'd go for the latter if you want original experience for sure.
You need the titanium version of eq which has 25 expansions. You can either buy it, or I provided a Google drive link on a previous page where you can just grab it. Then there's another link to the project 1999 site where you need to sign up and follow the getting started section.
As deekyfun says, it takes a while but you've only got to do it once and it's all free.
Then you have to sort out the ui once you're up and running but that's just personal aesthetics and trying to remember buttons and stuff.
If you do go on there, the "project 1999 server green" is the one you want to go on as it has an active community and economy

Is there some special set up needed for project 1999? I feel like I remembering it needing a specific version of EQ? You need 2 things 1, titanium edition of eq. This can be bought but I'm handily going to provide this link from reddit that has the whole thing available from a public Google drive - forums.mmorpg.com/discussion/459361/for-anyone-looking-to-join-project-1999-but-do-not-have-the-disks2, then you need to visit here and just follow the steps on the "getting started" option on the left hand side. Ah. I've got to that point in everquest where you have to hunt down your spells when you level up.
I haven't done this for 20+ years and had to plan the whole journey out, with boats, binding here and there, having a map as a guide, going across the continent etc.
Only to finally arrive and I don't have enough fucking money to get them all! Argh!

I haven't played it for a while now, but it was still very active when I last returned about a year ago. They are a little slow when it comes to releasing content but since the level cap has never increased and gear is different rather than straight up more powerful, every expansion's content is still somewhat relevant and active. I hadn't played since shortly before the first expansion released, so there was a gigantic amount of things to do.
I'd say it's a really great casual MMO. You can log in whenever you want and hop into some big meta events, dungeons or just do whatever you want solo, without fear of falling behind or missing out on anything. If you like more hardcore/progression stuff you'll probably run out of new things to do pretty fast.
I have no idea about the state of PvP and World vs World.
